Relevant conditions are as follows: ; � r 12.6. The Tenth Avenue ingress and egress shall be restricted the ;- combined 30 to 40 foot driveway/alley approach. 12.8. The Applicant shall, within six months following final approval of the special use permit, apply for a building permit. If an application for a building permit has not been submitted within the six-month time frame, the special use permit shall become null and void. All of the conditions necessary for receiving a Certificate of Occupancy shall be met within one year following the order of decision or the permit shall also become null and void. 12.10. The Applicant is to close off the driveway approach on Dearborn closest to 10t Avenue. Upon inspection of the Building Department files, the Certificate of Occupancy was issued 7-11-2003, Permit No. 30887.
