Linden Street Complainant(s): Planning & Zoning Department Complaint: Failure to comply with conditions set forth in issued and approved Special Use Permit No. SUP-257-05 relating to the initiation of child care operations. Linden Street Caldwell, ID 83605 INSPECTION RESULTS Page 2 2008-01-31-1015 02-06-2008 Records reflect that Temporary Certificate of Occupancy B-Com-40586-06 (attached to report) was issued on 03-16-2007 and expired on 05-31-2007. No record of further temporary of actual Certificate of Occupancy. SWA 02-07-2008 Site inspection was conducted. Contact was made with an unidentified employee who contacted the business owner, Mindy Johnson via a cell phone. I spoke with Mindy Johnson who advised that she has a new Temporary Certificate of Occupancy which will expire 03-31-2008. She indicates that by that date she will have the remaining work (paving) completed. She advised that she was working with Building Inspector Kevin Hinsdale on this matter. I inspected the property and found a sign (Photo 02) advertising the business with "Flexible Hours", contrary to condition 8.16 limiting the operation hours to 6:30 AM to 7 PM, Monday through Friday. SWA 02-13-2008 Kevin Hinsdale advised that he could not find a new Temporary Certificate of Occupancy on file. I issued a letter to the business owner requesting a copy of the new Temporary Certificate of Occupancy. SWA 02-15-2008 Received a telephone call from the business owner, Mindy Johnson, who insisted that there is a new Temporary Certificate of Occupancy. She also indicated that the final condition requirement, paving of the parking lot, would be completed at the end of March 2008. She was advised that there is still no record of a current Temporary Certificate of Occupancy and to contact the Building Safety Department to obtain a new one. SWA INSPECTION RESULTS Page 3 2008-01-31-1015 03-05-2008 Records reflect that a new Temporary Certificate of Occupancy (No. B- Com-40586 expiring on 05-31-2008) was issued on 02-25-2008. SWA 05-28-2008 Records reflect that the subject paving was completed and a Certificate of Occupancy for the Change-in-Use Permit was issued on 05-28-2008.
