EMPLOYEE BENEFIT PLAN
TRUSTEE
SPECIAL MEETING
HELD ON January 6, 2010
The meeting was called to order by ElJay Waite at 1:50 p.m.
Those in attendance: Monica Jones, E1Jay Waite, and Mary-Kaye Stewart
DISCUSSION ITEM(S):
• Reviewed the auditor's report and discussed the Significant Audit Findings.
Mary-Kaye was instructed to track separately the employee contributions and
• the employer contributions for the health and dental plans. By tracking
contributions separately, we will be able to determine the percentage that each
party is making at the end of each the fiscal year. MOTION by E1Jay Waite to
accept the audit report and forward to City Council for approval and review
SECONDED by Monica Jones. MOTION by E1Jay Waite to forward the audit
report to the Department of Insurance SECONDED by Monica Jones.
• The beginning balance of the Employee Benefit Plan Trust for 10/2008 was
approximately $619,000. As of 10/2009, the trust balance was approximately
$678,375.
• MOTION by Monica Jones to execute the Administrative Services Agreement
with Blue Cross as approved by City Council on January 5, 2010 and forward to
Blue Cross for signature SECONDED by ElJay Waite.
MOVED by ElJay Waite, SECONDED by Monica Jones to adjourn at 2:15 p.m. and meet
at the next Trustee Meeting scheduled for January 25, 2010 at 9:00 a.m.
Those voting yes: Unanimous.
